Housing Support (formerly known as GRH)
Housing Support is a program for people with disabilities and older adults who have low income. It helps pay for housing and things people need to stay housed in the community.
Greater Twin Cities Care will assists clients in obtaining Housing Support to ensure they have the necessary resources for stable and secure living conditions.
Eligibility
This category may include:
A General Assistance (GA) basis of eligibility, or
A Supplemental Security Income (SSI) basis of eligibility.
Individuals aged 18 or older with a certified disability or disabling condition, or those aged 65 or older.
Possessing low income and limited assets.
Residing in a setting authorized by the county or tribe with a Housing Support Agreement.
Housing With Services - Independent (HWS-I)
The Housing with Services Independent Program offers eligible individuals across Hennepin County the chance to find permanent housing options. Participants may also have access to additional services aimed at improving housing stability, supporting employment, or meeting health needs.
LONG TERM HOMELESSNESS (LTH)
Referrals for the Long-Term Homeless Program are submitted through Hennepin County’s Coordinated Entry for Single Adult Homeless individuals. Upon acceptance into the program, individuals will be placed in permanent housing at various locations across Hennepin County, based on their personal preferences. Program participants may also qualify for additional services that assist with housing stability, employment, or health needs.
